COVID Vaccination Status Questionnaire
How to use the new COVID Vaccination Status Questionnaire functionality when making a desk booking.
Select a date/time and location for your desk booking through either the web or mobile application.
Once you have successfully booked your desk, you will receive an email with the booking confirmation and COVID questionnaire. The email will look something like this.
Simply click the questionnaire link to view and complete your COVID questionnaire.
Note: The COVID Vaccination question will be sent once every 30 days after a desk booking is made.
